Families who wish to move forward with enrollment should contact the School’s Director and complete the enrollment package which includes: • Parent Questionnaire • Student Evaluation Form – Children who are transferring from another school should have the current teacher to complete the form. • Evaluation, Recommendation, and/or IEP – When applicable, documentation for children who have had evaluations, diagnoses, and education plans must accompany pre-enrollment packet. *Shot records will be due prior to the student’s first day of school.
